This form is to be completed by each local chapter Project Chairman.  These Projects are: Educational Loan Fund (ELF), International Peace Scholarship (IPS), Program for Continuing Education (PCE), Scholar Awards (PSA), and STAR Scholarship (STAR), These forms are due annually by March 1st (mail or email) to the State Project Chairmen listed at bottom of the form.
