The Gift Transmittal Form is completed by both individuals and Local Chapters when submitting donations (including but not limited to Love Gifts and annual Local Chapter project contributions) to the WV State Treasurer for processing. Submissions from individuals will be automatically credited to the individual’s local chapter reflected on the form. Please mail form and check to address on the Gift Transmittal form.
